The QHR and you

Your vital medical information will progressively become available
in your QHR.

Doctors, pharmacists, nurses and other authorized professionals will be able to view it by means of a secure network in regions where the QHR is implemented.

Imagine . . .

  • That you’ve just moved from Québec to Sherbrooke and you’re going to see a new family physician.
  • That you arrive, unconscious, to the hospital where you are immediately cared for by an emergency physician.

What would it be like if your health care providers could access your vital medical information in real time from their computers?

You’ll find out soon enough—that’s exactly what the Québec Health Record (QHR) makes possible!

 

 

 

What is the QHR?

 

Your QHR is not a card with a chip.

The QHR is a new tool for authorized health professionals participating in the QHR project, which was established by the Minister of Health and Social Services in the Capitale-Nationale, Lanaudière, Estrie and Montréal regions.

The QHR provides the health care professionals who are treating you with rapid access to the most important information about your health.

The information in your QHR

QHRs complement—but do not replace—the medical records kept by the health care facilities, medical clinics and pharmacies you visit.

You may have a QHR if you live in a participating region.

The QHR may eventually become available in other regions, to provide other Québec residents with the benefits of this tool. Information campaigns will be launched to keep you informed.

Your QHR will gradually be populated as participating facilities join the project. It will contain the following information:

  • Information about the prescription drugs you receive from a pharmacy connected to the QHR system;
  • The results of exams and lab tests that you have undergone in a health care facility, including tests you had as far back as November 2010;
  • The results of medical imaging exams that you have undergone in a health care facility, including exams you had as far back as February 2011.

If you agree to have a QHR, your doctor will be able to prescribe your medication electronically. When you give the pharmacist a print-out of your prescription, he or she will be able to access the prescription electronically. The use of electronic prescriptions can help to reduce the number of requests pharmacists must make for clarifications on prescriptions from clinicians.

This information will be available only to authorized professionals who practice at participating health care facilities, such as pharmacies, medical clinics, family medicine groups, CLSCs and emergency rooms.
