Right to request the correction of inaccurate information
If you consult the Québec Health Record (QHR) and notice mistakes or inaccurate information, you have the right to request a correction.
In order to do this, you must send and complete the correction request form to the agent responsible for the access to documents and protection of personal information. Your request must be submitted along with a copy of a valid piece of signed photo I.D.
The agent has 20 days to process your request which may be extended by 10 days upon notification.
The decision of the agent responsible for the access to documents may be appealed to the Commission d’accès à l’information.
